NJ Amends PIP Provider Treatment Plan Form

On February 9, 2016, Acting Commissioner of Banking and Insurance Richard J. Badolato issued Order No. A16-101, amending New Jersey's Attending Provider Treatment Plan (APTP) form used by providers when submitting treatment and testing requests to automobile insurers for personal injury protection insurance coverage. The revised APTP form addresses the tenth revision of the International Statistical Classification of Diseases and Related Health Problems and further seeks to assist the New Jersey Department of Banking and Insurance in data collection.
